WATERBURY HEALTH PRIMARY AND PEDIATRIC LOCATIONS ACHIEVE PATIENT-CENTERED MEDICAL HOME RECOGNITION

Six Waterbury HEALTH primary care locations in Middlebury, Naugatuck, Woodbury and Watertown have achieved Patient-Centered Medical Home Recognition by the National Committee for Quality Assurance.

This program highlights medical practices who demonstrate they provide high-quality patient-centered primary care. Alliance Medical Group, part of Waterbury Health, offices and their physicians and staff have been recognized as Patient-Centered Medical Homes since 2017. The following locations received the NCQA Patient-Centered Medical Home Recognition:

  • Middlebury Internal Medicine 1625 Straits Turnpike
  • Middlebury Internal Medicine – 10 N. Benson Rd
  • Naugatuck Internal Medicine, 305 Church Street
  • Woodbury Internal Medicine, 40 N. Main St.
  • Watertown Internal Medicine, 777 Echo Lake Road
  • Pediatrics, 1625 Straits Turnpike, Middlebury

 

Each day Alliance Medical Group clinicians and teams care for more than eight hundred patients.
