UConn Health Community Network offers home health care to our patients who wish to recover and rehabilitate from an illness, injury, or surgery in the familiar surroundings of their home. UConn Health Waterbury Health at Home, a wholly owned affiliate of UConn Health, is a private home care agency that has served the Greater Waterbury region for over 80 years.  

Our multidisciplinary team includes skilled nurses, therapists, social workers, and home health aides who deliver high-quality, compassionate care to every patient. We carry out personalized treatment plans that meet patients’ specific needs and help them regain their health and independence.  

UConn Health Waterbury Health at Home has been awarded the 2019 Patient Satisfaction Award of Distinction by Fazzi Associates, which recognizes the top 25% of home health organizations for superior performance in patient satisfaction based on the results of a national, standardized survey.
